INSTRUCTIONS FOR COMPLETING THE BULK MEMBERSHIP FORM FOR THE PROFESSIONAL ASSOCIATION (TPA):  MUST BE FOR 5 OR MORE MEMBERSHIPS

1)       DOWNLOAD FORM FROM THE TPA WEBSITE CLICK HERE.

2)      OPEN FORM IN EXCEL

3)      ON THE SECOND LINE OF THE FORM PUT THE ORGANIZATION’S INFORMATION.  BE SURE TO INCLUDE ALL OF THE FOLLOWING INFORMATION:

a.       COMPLETE NAME OF ORGANIZATION

b.      STREET ADDRESS OR PO BOX OF THE ORGANIZATION

c.       CITY

d.      STATE

e.      ZIP

f.        AREA CODE

g.       PHONE NUMBER

4)      BEGINNING ON THE 3RD LINE OF THE FORM COMPLETE EACH LINE FOR ALL EMPLOYEES YOU ARE ENROLLING INCLUDING:

a.       LAST NAME

b.      FIRST NAME

c.       USER NAME (THIS CANNOT BE THEIR FIRST & LAST NAME)

d.      EMAIL ADDRESS

e.      MEMBER TYPE CODE—EXECUTIVE PROFESSIONAL ($125); FULL TIME ($60) OR PART TIME ($15)

f.        NEW OR RENEWING MEMBER (THIS WILL DETERMINE HOW IT IS TRANSFERRED INTO THE SYSTEM AND MUST BE COMPLETED)

g.   SELECT THE REGION FROM COLUMN O FOR EACH MEMBER

h.   SELECT THE CHAPTER FROM THE GROUP CODES COLUMN P FOR EACH MEMBER

i.       NOTE:  THE ORGANIZATION AND ADDRESS CAN BE LEFT BLANK ON THE INDIVIDUAL’S AS IT WILL AUTOMATICALLY BE UPLOADED INTO THE SYSTEM.  BE SURE IT IS CORRECT ON LINE 2 AS THAT WILL BE THE ADDRESS ASSOCIATED WITH EVERYONE.

5)      ONCE THE EXCEL FORM IS COMPLETE, SAVE IT AND SUBMIT YOUR SAVED EXCEL FORM BELOW.

6)      TIM WILL UPLOAD THE FORM, ESTABLISH THE MASTER ACCOUNT, WHICH IS THE ORGANIZATION, GENERATE AN INVOICE, AND SEND YOU THE LOG IN INFORMATION TO PAY THE INVOICE WITH A CREDIT CARD.  YOUR BILL WILL SHOW A 20% DISCOUNT ON YOUR MEMBERSHIPS.
